Haemorrhoids

Haemorrhoids may be defined as vascular engorgements of the haemorrhoidal plexuses in the submucosa of anal canal beneath the mucocutaneous lining

Internal haemorrhoids - its involves the upper 2/3 part mucosal line part of anal canal above dentate line in the superior haemorrhoid plexuses

External haemorrhoids - it occurs beneath the cutaneous lining of lower1/3 rd part of anal canal is formed by inferior and middle haemorrhoidal plexuses

Externo-internal haemorrhoids - After long time straining enlarged internal haemorrhoid with external haemorrhoidal plexuses protrude at anal orifice,this is called externo internal haemorrhoids

DEGREE OF HAEMORRHOIDS

FIRST DEGREE HAEMORRHOIDS

During defecation the pile mass descent down in the lumen of anal canal but not outside the anal orifice

SECOND DEGREE HAEMORRHOIDS

In this stage the pile mass becomes larger in size and descents down outside the anal orifice during defecation and reduced itself in the anal canal after straining act is over

THIRD DEGREE HAEMORRHOIDS

In this stage the pile mass protrudes at anal orifice during defecation,it does not reduce itself but needs reduction by digital manipulation.it may reprolapse again even with slight straining or exerstion and needs digital reduction in the anal canal

FORTH DEGREE HAEMORRHOIDS

In advanced cases internal haemorrhoids with external haemorrhoids plexuses gain a larger size and remain permanently prolapsed at anal orifice,these pile can not be returned into anal canal

Fissure

An anal fissure may be defined as crack or vertical ulcer in the cutaneous lining of lower anal canal from anus outer end to reach anus inside 2c.m.

Majority of anal fissure occurs in medline posteriorly,IN female its occurs anteriorly

This position is called medically 12’o’ clock and 6’o’clock

Fistula & Abscess

Fistula

A fistula is a tubular structure which connects two epithelised cavities with each other or one cavity to the surface

Types

  • Subcutaneous-fistula : tract lies in the lower part of cutaneous lining of anal canal
  • Low anal fistula : fistula open at lower anal canal
  • High anal fistula : fistula tracts passes upwords thr.the anal sphincters and ends up the the level of ano rectal ring
  • Anorectal fistulae : anorectal or pelvirectal fistula tract in these cases may extend beyond the anorectal junction and cavity lie even above the levator ani muscle

Symptoms

  • Pus
  • Discharge from boil [blood +pus]
  • Pain during defecation
  • Fever
  • Swelling
Abscess

Anal gland got infected at anal region,it is called as Anal abscess

Types

  • Perianal abscess : when pus spreads between two sphincters to form erythematous bulge at anal verge
  • Ischiorectal abscess : abscess formation in ischiorectal fossa and levator ani slop upward so abscess quite large
  • Submucosal or intramuscular : presents with pain in a patient's rectum and no external swelling, unless it is complicated by an ischiorectal or perianal abscess. On rectal examination you may be able to feel a soft, diffuse, tender swelling extending upwards from his pectinate line.
  • Pelvirectal : Pelvirectal presents with fever, but no local anal or rectal signs Later, it may extend downwards into his ischiorectal fossa. With your finger in his anus, you may be able to feel fluctuation above and lateral to his anorectal ring.

  • Pus from Anus
  • Induration or fluctuation

Pilonidal Sinus

Pilonidal sinus is an infected tract under the skin between buttocks.Those persons who had to be travel constantly for long period in jeep .

so it is called as jeep disease. it was found in army person during second world war in 1935-45

Symptoms

  • Pain after sitting or standing
  • Reddened sore skin around area
  • Pus or blood draining from abscess
  • Faul odour from wound
  • Bulk of hair protruding from the lesion
  • Formation of more than opening tract or holes on skin

Rectal prolapse

Circumferential descending down of mucosa or entire thickness of rectal wall outside the anal outlet is called rectal prolapse.

Types

  • Concealed : the prolapsed doesnot descent down beyond the anal orifice
  • Incomplete : the prolapsed is less than 4c.m.and consists of double layer of rectal mucosa which palpable by finger.its reduced smoothly or digitally pressed
  • Complete : Total thickness of rectal wall protrudes out of the anal orifice

Symptoms

  • Protrusion of mass during defecation which is replaced digitally or automatically after the act
  • Underclothes are soiled with mucous
  • Blood discharge from anus
  • In per rectum examination double ring of mucosa fold found

Anal Veneral Disease

Gonorrhal proctitis-

Infective organism is nisseria gonorrhoea ,which is transferred to anal region by and infected homosexual during intercourse.

Symptoms

  • Intense itching at anal and perianal region
  • Mucopurulent discharge from anus
  • Tenesmus and bleeding per anum
  • Joint pain
Anal Syphillis-

This disease is transmitted after intercourse in homosexual.

Symptoms

  • Chancre at anal region painless
  • Inguinal lymph gland enlarged and painless
  • Infected fissure at 6’o’ clock
Herpes Simplex Virus-

It is complication of hiv infection,occur due to homosexual,its viral infection.

Symptoms

  • Viscles and postules,shallow ulcers at perianal region
  • Burning pain,mucoid,bloody discharge from ulcers
  • Ulcer persistant ,non healing,painful
ANAL CONDYLOMATA (PAPILLOMAS)

Anal papillomas is warty growths commonely seen in anorectal clinics.

Symptoms

  • Warty growth non malignant,its look like cauliflower
  • Itching
  • Bleeding
  • Foul discharge from anal region
  • Warty growth envolving the complete surface of anal orfice and perianal region
  • Wart Friable and bleed easily
PURITUS ANI

Chronic anal fissure and prolapsed piles with traces of anal discharge causes pruritus ani.

Symptoms

  • Anal cracks extends radially to anal canal
  • Tenderness is absent
  • Anal itching
  • Redness of skin around the anus
  • Itching is often intense and worse at night
  • Chronic inflammation
  • Fungal infection
